Stockport Together is the name given to the activity being carried out in partnership between Stockport NHS Clinical Commissioning Group, Pennine Care NHS Foundation Trust, Stockport Metropolitan Borough Council, Stockport NHS Foundation Trust (Stepping Hill Hospital), and Viaduct Health.

The aim is to reform the way health and social care is delivered in Stockport, to provide the best possible outcomes for local people at a time of growing demand and constrained funding.

We need your help to shape the plans that will ensure the people of Stockport continue to receive the best quality of care in the most appropriate setting.

If you sign up to Stockport Citizens you may receive information about the transformation programme and be invited to take part in surveys, focus groups and attend engagement events.
