We are arranging a further event in this Neighbourhood and then approach the charity (Contact the Elderly) to gain permission/obtain funding for further Neighbourhoods.

Overview

In July 2019 a team of clinicians and social care staff highlighted isolation and loneliness issues in older people as the focus in the Bramhall & Cheadle Hulme Neighbourhood ‘Better at Home Plan’.  In August, the Contact the Elderly charity were contacted regarding their scheme ‘Tea with the GP’ as a way to assist with isolation issues in the community.

 

Why We Are Consulting

 

The aim of the ‘Tea with the GP’ scheme is to partner with GP practices to bring the model into GP surgeries as a way of appropriately and discreetly monitoring individual’s health and wellbeing as well as creating a greater sense of community between staff and patients.
