As you may know, Stockport Together* is looking at making some changes to health and social care in Stockport, and we want to work with our local people to bring these about. Below is a short summary of some of the changes we are working towards:

  • Arranging health and social care services around teams within Stockport’s neighbourhoods, putting GPs at the centre. These teams will also include district nurses, social workers and community mental health teams. Teams will draw on support from the voluntary and sector community for lifestyle and wellbeing support.
  • Changes to the way we provide outpatient care and urgent care
  • Increased prevention, empowerment advice and support to allow people to manage their own conditions
  • Changes to intermediate tier services (these are a range of services in place to help people who do not need to be admitted to hospital, but need some support to remain independent).

We are keen to ensure as many people as possible have the opportunity to listen to and influence our plans in more detail.


Why we are consulting

To help with this we have arranged a number of Listening Events around the borough to talk to people about the changes and to hear their views.
